Metabolic dysfunction is the upstream cause of most chronic disease. By the time the standard physical catches it (HbA1c > 5.7%, fasting glucose > 100), the damage has been building for years. What goes into the score

The composite weights five domains:

  1. Fasting glucose and insulin — the foundation. HOMA-IR derived from your fasting numbers.
  2. HbA1c — 90-day glucose average; weighted lower if you have anemia or hemoglobinopathy
  3. Triglyceride-to-HDL ratio — a free, derived insulin-resistance index
  4. Waist circumference and body composition — visceral adipose is the largest correlate
  5. ApoB — downstream cardiovascular consequence

The output is 0-100. Above 80 is excellent; below 50 is metabolically dysfunctional and warrants intervention even if no clinical diagnosis applies.

Why a composite beats single markers

Insulin resistance is a spectrum, not a yes/no. Many men with “normal” fasting glucose have insulin levels that are already 3x optimal. The composite catches dysfunction earlier than any single marker.

Your protocol

After scoring, the tool returns a prioritized list:

  • Highest-leverage intervention for your specific pattern (e.g., zone 2 if low VO2 max, fiber and resistance training if high HOMA-IR, statin discussion if elevated ApoB)
  • A 90-day retest plan
